Broken Glass Box Disposal Request

This form is for broken glass boxes requiring disposal. Standard TCU Facilities pickup time is typically 2-3 business days.

Prior to Request Submission:

1. Make sure the inner liner is sealed inside the box and the outside lid is taped securely to the box.
2. Place broken glass box in designated pick up location.

Notice: Submitting an incomplete request, or failing to properly seal your containers prior to the arrival of Facilities, may result in the rejection or delay of your pickup request.
