Chemical Pickup Request Form

Please submit this form exclusively for the collection of chemical waste. Environmental Health & Safety (EHS) standard response time for waste collection is typically 2-3 business days.

Submission & Packaging Requirements:

• Proper Labeling: All containers much have a complete waste label. Chemical contents must be written out completely - do not use abbreviations (e.g., "EtOH") or chemical formulas (e.g., "H2SO4").
• Proper Packaging: Ensure all containers are tightly closed with a screw-on cap. Flask stoppers, foil, and parafilm are not acceptable closures.
• Other Hazards: To request a pickup for biological or radiological waste please visit the Waste & Environmental page.

Notice: Submitting an incomplete form, or failing to properly cap and attach your pickup label prior to the arrival of EHS, will result in the rejection or delay of your pickup request.
